The staff will listen carefully and respond promptly to your questions and concerns. Patients find Dr. Frazier is approachable and appreciate her clear, detailed explanations of what she can do for them and why. Everyone takes pride in giving you honest, delicate, professional treatment.

A COMMITMENT TO CARE

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, treatment can be done the same day as the consultation. However, a complex medical history or extensive treatment plan may require an evaluation and a second appointment to provide treatment.

Please assist us by providing the following information at the time of your consultation:

X-rays - if your previous dentist has taken x-rays within the last 12 months, you may request that they forward them to our office a week prior to your appointment. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.
A list of medications you are presently taking
If you have dental insurance, bring the necessary card and identification (driver license or id card). This will save time and allow us process any claims.

IMPORTANT: A parent or guardian must accompany any patient(s) under 18 years of age.

Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, artificial heart valves and joints, heart murmur, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)
